Enrollment Forms

Parents of all new and returning students need to complete Enrollment Forms, for each student, and submit to the school as soon as possible. Enrollment forms can be found and submitted under the "files and forms" page within Connect2Prep
All new day students and domestic residential students must submit the following forms:

  1. Enrollment Contract
  2. Consent for Treatment & Emergency Contact information
  3. Student Health Information Form
  4. Physical Form and Immunization Form
  5. Science Lab Safety Contract
  6. Residence Information & Policies Booklet (boarding students only)

Optional Forms – submit if applicable to student

  1. Minnesota State High School League Form
  2. Doctor’s Order Form
  3. Self-Administration of Medication Authorization 

Please note:  Each student MUST have his/her enrollment forms submitted to the school before school starts.  I-Pads will be not distributed, and students will not be able to attend classes, unless all forms are received.

All students are encouraged to wear Saint John's Prep clothing to show their school pride. To see the selection and to order shirts, pants and gear, click here.

The Upper School (Grades 9-12) does not have a specific school supply list. However, it is recommended to have folders, a water bottle, notebooks, pens, highlighter markers, and pencils available. Faculty will review class syllabus and supplies needed, if any, during the first week of school. 

All students are encouraged to have a refillable water bottle for use with the school's water hydration stations.This sustainability effort will help reduce the amount of plastic bottles consumed in our community.
