If your child/ren needs to take medication during the school day, the following forms must be printed off, completed, signed and returned to the school nurse.
Below are the immunization and physical forms. These forms must be printed off, completed, signed by the doctor and returned to the school nurse. The forms used at the doctor's office are also acceptable.
